“Practical Implementation of Population Management for Improved Patient Outcomes”

The U.S. healthcare system is evolving from a volume-based reimbursement model to one focused on the value of care provided.  The Georgia Hospital Association’s Compass Practice Transformation Network hosted its annual meeting, where participants continued to develop an understanding of the value-based reimbursement system, and learned best practices to succeed.  The event took place May 3, 2019, in Macon, GA.

PYA Senior Consulting Manager Allison Wilson presented “Practical Implementation of Population Management for Improved Patient Outcomes.”  The presentation outlined guidelines for population health management, which included:

  • Aggregating patient data to improve both clinical outcomes for patients and the financial outcomes for care providers.
  • Revising internal processes and resources to cost-effectively improve the health outcomes of a specific population, while empowering patients to better manage their health.
