PYA Releases Updated Chronic Care Management Manual

PYA has released an updated chronic care management (CCM) manual that reflects new 2017 CCM billing rules and offers guidance to providers as they develop and implement compliant CCM programs.

Since Medicare began reimbursing for chronic care management (CCM) services in 2015, providers have continued to seek comprehensive and fail-safe guidance for structuring, implementing, and managing CCM programs.  With the 2017 revisions to CCM billing rules, this updated manual is a timely resource for providers navigating these requirements.

PYA’s updated Chronic Care Management Clinical Services Manual instructs providers in developing efficient and regulatory-compliant CCM programs.  A valuable resource for providers in understanding Medicare billing and compliance rules for CCM, it provides sample policies, procedures, and forms to help implement effective CCM.
