CMS TEAM Participants Take Note: FAL and CEL Documentation Due December 31, 2025

In addition to participant track selection and alternative payment model options documentation, the Centers for Medicare & Medicaid Services (CMS) announced it is requiring TEAM participants to submit documentation regarding collaborators and collaborator agreements by December 31, 2025.

What documentation is required?

CMS is now collecting Performance Year 1 deliverables. By December 31, 2025, TEAM participants must submit via the TEAM portal a Financial Arrangement List (FAL) and Clinician Engagement List (CEL):

  • A FAL identifies providers or suppliers engaged in financial arrangements with the TEAM participant or its collaborators.
  • A CEL identifies eligible clinicians engaged in TEAM activities and in a contractual relationship with the TEAM participant for the given quarter.

What if I don’t have financial arrangements or engaged clinicians?

If a TEAM participant has no financial arrangements or engaged clinicians to report, the participant must attest to this status by December 31, 2025. TEAM participants will submit FAL/CEL documentation or submit the attestation quarterly thereafter.

What data is required?

In addition to provider-identifying information, TEAM participants will provide specific data about financial arrangements/engaged clinicians on the FAL/CEL form including

  • Relationship to TEAM participant: TEAM collaborator, collaboration agent, downstream collaboration agent, clinician
  • Financial arrangement or contractual relationship start date and end date
