PYA White Paper Provides 2025 Update on Medicare Billing Rules for Care Management

PYA has released an updated and expanded white paper, “Providing and Billing Medicare for Care Management Services.” This is the 10th year PYA has provided the in-depth resource, which now has been expanded to include:

• Chronic care management
• Complex chronic care management
• Principal care management
• Advanced primary care management
• Care plan development

PYA first published its chronic care management (CCM) white paper in 2015, when Medicare began reimbursing physicians for these services. As the Centers for Medicare & Medicaid Services (CMS) has updated billing rules and expanded reimbursement, PYA has updated this resource on which numerous providers have relied in developing their care management services. The 2025 version of the white paper reflects the most recent round of rulemaking and guidance relating to numerous care management services, as well as updated payment rates.
